Travel - Temporary Assignment
Start date: 10/26/2026 | Duration: 13 weeks | Weekly pay: 1,826 – 1,878 per week | Guaranteed hours: 40 | Shift: 8:00 a.m. to 4:30 p.m., Monday–Friday
Assignment setting: hospital-based medical social work in an acute hospital environment

Responsibilities
- Conduct psychosocial assessments for patients and families from admission through discharge.
- Lead discharge planning and care coordination to ensure safe and timely transitions to next care settings.
- Provide counseling and psychosocial support to patients and families coping with illness, hospitalization, or related stressors.
- Collaborate with the medical team, case management, and community resources to address barriers to care, including planning for post-discharge needs.
- Identify and arrange appropriate community services, home health, palliative/hospice referrals, and placement as needed.
- Document assessments, care plans, and interventions in the patient record, adhering to hospital policies and regulatory requirements.
Requirements
- Active Georgia Licensed Clinical Social Worker (LCSW)
- AHA BLS certification
- Minimum 2 years of medical social work experience in an acute hospital setting
- Ability to work effectively in a hospital-based environment and collaborate with interdisciplinary teams
- Availability to work the shift and holiday coverage described above
